Please review this sheet prior to registration. Some genres require concurrent enrollment in other genres. Dancers must meet the level prerequisite to enroll in the class.

Read and agree to our Studio Policies & Conduct by checking the appropriate boxes.
Payment is due in full at the time of registration or classes will be automatically dropped from your account. Please note that even when a $0 balance is due, you must complete the process.
Please register on your assigned registration day only. Classes registered for on prior to your assigned day(s) will be cancelled.
We kindly ask that if the end-of-year recital presents a conflict and your dancer will not participate, that they be enrolled in a technique class.

METHODS OF PAYMENT
We accept personal checks, cash, MasterCard, Visa. (Please note, we do not accept American Express or Discover.)
Easiest Way to Pay and its FREE!
Sign up today, and never worry about late fees again! Tuition payments can be automatically charged to your credit card, eliminating the need to write a check each month. Log-in to your online studio portal and select the Auto-Pay option. Please call or stop by the office if you have any questions about using Auto-Pay.
A $25 non-refundable registration fee per student is due and payable upon enrollment in a class. This fee carries through until the following school year session.
Tuition payments are due and payable on the 1st of the month. Tuition fees are based on an 11-month installment plan, and the same fee is charged each month, regardless of how many classes were held in that month.

Accounts are considered past due on the 8th of the month. A $20 late charge will be added after the close of business on the 7th of each month in which your payment is past due. A $25 returned check charge will be added for any returned check. After the second returned check, services will only be payable by cash or credit card. Costume Fees for Performing Dancers will be due in January 15, 2020.
DROPPING A CLASS
Class Drops and/or Account Withdrawals must be done in-studio at the Front Desk. Drops/Withdrawals must be done on or before the 30th of the month to discontinue tuition charges for upcoming months. Drops/Withdrawals are considered complete when the Account Holder completes the Drop Form provided by the studio. If a Drop Form is not completed by the due date, the account will be charged the next month's tuition, no exceptions.

Tuition remains the same each month regardless of whether it is a 5-week month or shortened month due to holidays. Dance classes will run from August 2018 through June 2019.
Unlimited Dance Pass (Single Dancer) applies to a single dancer enrolled in a minimum of 8 class hours per week at Impact Dance Center.
Unlimited Family Pass applies to one or more siblings enrolled in a minimum combined 11 class hours per week at Impact Dance Center.
